Health Forms

Scouts and adult leaders are required to provide a health form for every event. The best advice is to fill out the PDF form electronically, and then print/sign a copy whenever one is needed. That way you don’t have to fill out the same info multiple times.

Parts A & B are the only ones needed for weekend camping trips less than 72 hours. Week-long events (Summer Camp) and high-adventure camps require Part C, which requires a doctor’s signature.

Applications

One of the first things that need to be done when joining our troop is to fill out a BSA Youth Application. For new Scouts, this will ensure that you are registered with the BSA. For existing Scouts crossing over from a Pack, or transferring from another troop, completing the application will ensure that your transfer is registered correctly. This also ensures that the Scout will show up in ScoutBook, our online advancement tool.
